We focus on the areas where we can make the biggest difference. That means listening to local people, co-designing solutions, and delivering projects that really matter.
With residents at the heart of everything we do, we actively engage people to make sure health and care works for everyone in Lambeth.
Working with communities
When it comes to staying well, everyone has different needs. And not everyone can easily find and navigate the support there is. We work side-by-side with residents, community groups, and grassroots organisations to create local solutions.
Projects include: The Lambeth Together Health and Wellbeing Bus, Health and Wellbeing Hubs and Thriving Communities
Living well for longer
There are many health conditions that are preventable or treatable with early help. We’re working together to support more people to manage and prevent long-term health conditions so they can live full, active lives.
Projects include: Chronic Pain, Hypertension (high blood pressure), Cancer Screening and Diabetes.
Connecting services
We want to improve people’s experience of their local health and care services. We’re actively joining the dots so residents can access the right support, at the right time, in the right place.
Projects include: Ageing Well, Integrated neighbourhoods, Hospital@Home (also called ‘virtual wards’ – providing hospital care at home) and Chronic Kidney Disease.
