    Social deprivation and multimorbidity

    Research on Lambeth residents has shown that social deprivation and ethnicity have a significant influence on multimorbidity, which in this study was defined as having three or more of 12 long term conditions.

  2. Guys and St Thomas (GSTT) Transformation Team, Lambeth Council and CCG commissioners have been working together to help support and develop the commissioned community support (home care) providers.

    Community health and social care teams are frequently supporting the same individual at home and should be able to contact each other easily in order to improve the service they are jointly providing.

    Age UK Lambeth has started a resilient ageing project to help us all listen to each other and then act together: appreciating and doing more with what’s good about ageing in Lambeth, preventing and dealing with problems, and increasing everyone’s wellbeing across the borough.
